Required Parent Lab Kit Agreement/Liability Waiver

In order for students to be able to do real lab work at home, lab supply kits will be provided to each student. These kits may contain toxic chemicals, sharp objects, small objects, etc. Since the teacher will not be physically present with these supplies, the parent must assume the responsibility for the proper care and storage of the lab supplies in the kit. Before receiving the lab supply kit, a parent or legal guardian must complete the Parent Lab Kit Agreement/Liability Waiver below:
